Abstract:
Secondhand smoke at home affects family health. This mixed method design aimed to explore community perspective on smoke-free home and to assess the effect of community mobilization intervention for smoke-free home. Two communities were randomly selected to be the intervention and comparison group. Qualitative study was employed with the intervention group by using focus group interview with community committees, family members, and current smokers to explore community perspectives and raise awareness on smoke-free home. The community mobilization plan for smoking free home was implemented for the last 6 months by a quasi-experimental design with two groups pre and post-tested. Fifty-four families with current smokers inside home of the two selected communities were randomly selected with 27 families in each group. Data were collected by structured interview where question, were constructed by the researcher. The qualitative information was analysed by content analysis, while quantitative data were analysed by descriptive statistics, chi-square, paired t-test, independence t-test, and Fisher's exact test. The results showed that the community had negative attitude toward cigarette smoke, and they agreed to ban smoking at home. Also, family's negotiation skills should be strengthened, on the community's implementation plan for a smoke free home. After 6 months, the community participation in the intervention group was significantly higher than the comparison group (t=4.06, P<.001). Emotional support for non-smoking inside home (=5.39, P<.001), negotiating for smoking ban at home (t=8.20, p<.001), and smoke-free home rule (81.5%, X=30.0, p<.001) of families in the intervention group were significantly higher than the comparison group. Perceive severity of secondhand smoke (t=5.12, p<.001), perceive self-efficacy for non-smoking inside home (t=5.26, p<.05), perceive emotional support for non-smoking inside home of current smokers (t=5.80, P<.001) were significantly higher than the comparison group. Smoke-free home in the intervention community was increased to 77.8% which was significantly higher than baseline and higher than the comparison group. Findings of the study suggest that a family member is the key person that should be empowered for a smoke-free home. Public health nurses should work as collaboratively with community people in mobilizing them in the implementation of smoke-free home that would greatly improve community's health and sustainability.
